***Any food vendor who is not currently registered or permitted with Alamosa County Public Health must complete and submit a Temporary Food Vendor Application directly to Alamosa County Public Health before the event. For questions about public health certificates or application submissions, please contact Alamosa County Public Health; call 719-589-6639 or visit https://www.alamosacounty.org/182/Public-Health. Once your application is approved, please upload or submit your certificate at the bottom of this page.

***Any vendor who is not currently registered to collect and remit City of Alamosa sales tax must complete and submit a City Sales Tax Registration Form directly to the City of Alamosa Finance Department prior to the event. Vendors NEED to be registered through the State as well. If not, please start with the state, as a state issued buisness TIN is needed for approval.
